Sign up for the Sedgwick Recreation Fitness Center.  Follow these steps...

1.  Mail completed form with 1st months fee to:    SRC  PO Box 45  Sedgwick, KS  67135

2.  Please allow 7 days for card activation

3.  Someone with SRC will contact you about setting up a fitness center orientation

Monthly: Single non-student $20.00
Student $15.00
Single Senior (60 & over) $15.00
Senior Couple (60 & over) $25.00
Family $30.00

1 month free with 6 month membership (pay for 5)
2 months free with 12 month membership (pay for 10)
